What Is Otezla (Apremilast)?
Otezla is the brand name of the prescription drug apremilast, which is used to treat psoriasis and psoriatic arthritis.
Otezla is in a class of drugs known as phosphodiesterase inhibitors. It works by blocking the action of an enzyme in the body that causes inflammation.
The medication is manufactured by Celgene Corporation. The Food and Drug Administration (FDA) approved Otezla in 2014.
In clinical trials, about 31 percent of patients taking Otezla experienced a 75 percent improvement in the severity of their psoriasis symptoms after four months.
Otezla is also being tested for treating other chronic inflammatory diseases like rheumatoid arthritis, ankylosing spondylitis, and Behcet's disease.
Otezla may affect your mental health in unexpected ways.
Tell your doctor if you have or have ever had depression, mood disorders, or suicidal thoughts before taking Otezla.
You or a family member should alert your physician right away if you experience worsening depression, unusual behaviors or moods, or suicidal thoughts while taking this medicine.
Before taking Otezla, you should also tell your doctor if you have or have ever had kidney disease.
Otezla can cause weight loss. In clinical trials, nearly 12 percent of people taking Otezla reported unexplained weight loss of 5 percent to 10 percent.
You and your healthcare provider should check your weight on a regular basis. If you are losing weight, you should let your doctor know.
Otezla controls the symptoms of psoriatic arthritis, but it does not cure it. Continue to take this drug even if you feel well. Do not stop taking Otezla without first talking to your doctor.
Pregnancy and Otezla
Otezla could harm an unborn baby, according to some studies.
Upon approval of the drug, the FDA required a pregnancy exposure registry to assess the risks Otezla has on pregnant women who take it.
It's not known whether Otezla passes into human breast milk.
Before taking Otezla, talk to your doctor about the risks, and mention if you're pregnant, might become pregnant, or are breastfeeding.
Otezla Side Effects
Common Side Effects of Otezla
You should tell your doctor if any of the following side effects are severe or do not go away:
- Stomach pain
- Sore throat, cough, or fever
- Runny nose, sneezing, or nasal congestion
Severe Side Effects of Otezla
You should contact your doctor right away if you experience any of the following severe symptoms:
- Mood changes
- New or worsening depression
- Suicidal thoughts
- Unexplained weight loss
You should tell your doctor about all prescription, non-prescription, illegal, recreational, herbal, nutritional, or dietary drugs you're taking especially:
Otezla comes in a 30 milligram (mg) tablet form and is usually taken twice a day.
Your doctor will likely start you on a low dose of this medicine and gradually increase it each day for the first five days.
The following is a typical dosing schedule for the first five days of treatment:
Day 1: 10 mg, once a day (AM)
Day 2: 10 mg, twice a day (AM and PM)
Day 3: 10 mg, once a day (AM); 20 mg, once a day (PM)
Day 4: 20 mg, twice a day (AM and PM)
Day 5: 20 mg, once a day (AM); 30 mg, once a day (PM)
30 mg, twice a day (AM and PM)
You should take your pill around the same time each day with or without food. You should swallow the tablets whole. Do not split, crush, or chew them.
If you suspect an overdose, you should contact a poison control center or emergency room immediately.

Missed Dose of Otezla
If you forget a dose of Otezla, take it as soon as your remember.
However, if it's almost time for your next dose, skip the missed dose and continue on your regular medication schedule.
Do not double up on doses to make up for a missed one.
